Players are welcome from Union Township and all other surrounding communities. We are a Recreational League and all kids play! Season starts in April. Limited travel. A copy of ALL player’s birth certificates must accompany this registration form. Age level is determined by the player’s age for Softball on January 1, 2020. Requests made for players to “play up” a level will be decided by the league board.

Make checks payable to UFAAA. One check may cover multiple player registrations from the same family. A $50.00 refundable concession deposit per family is required and must be a separate check dated 4/1/2020 as it will be held and returned after 2 hours of volunteer time/child is completed. 

Late registrations will be accepted on a need basis only. No refunds after March 1, 2020 without board approval.

Parental/Guardian Authorization / Medical Release for Participation in UFAAA, Inc. Photo Release for Minors 

I, being the Parent/Guardian of ________________________________(print player’s name) hereby consent that the videotapes, photographs, and/or motion picture film and/or audio recordings made of his/her voice may be used by the Union Finley Area Athletic Association, Inc., UFAAA, Inc., their assigns or successors, in whatever way they desire, including television, electronic uses, newspapers, promotions, etc; furthermore, I hereby consent that such photographs, films, and recordings, and the plates and/or tapes from which they are made shall be their property, and they shall have all the right to sell, duplicate, reproduce, and make other uses of such photographs, films, recordings, plates, and tapes as they may desire free and clear of any claim whatsoever on my part. 

(Please initial) _____I assume all risks and hazards incidental to such participation, including transportation to and from activities, and do hereby waive, release, absolve, indemnify and agree to hold harmless the local league organization, UFAAA, Inc., the organizers, sponsors, supervisors, participants and persons transporting the player to and from activities for any claim arising out of injury to the player. _____I further agree to furnish a birth certificate for the player upon request of league officials. I do agree my child will play with any team to which he/she is assigned by league officials. If I elect not to permit my child to play on said assigned team, I understand that all registration fees will be forfeited. I understand that no uniform will be provided to any player until all payment obligations are made in full. _____I understand that families are required to work at least 2 hours in the concession stand per child and team and any playoff games and tournaments as required by the league. _____I understand that there will be no registration fee refunds after March 1st without board approval.
